What Is a Patient Counseling Script?

A patient counseling script is a structured guide that pharmacists use when educating patients about new or existing medications. Effective counseling covers the medication's purpose, proper dosing and administration, potential side effects, drug and food interactions, storage requirements, and what to do if a dose is missed. Many states require pharmacists to offer counseling on every new prescription.

Under OBRA '90 (Omnibus Budget Reconciliation Act of 1990), pharmacists are required to offer counseling to Medicaid patients for new prescriptions. Most state boards of pharmacy have expanded this requirement to cover all patients. Failure to provide adequate counseling can result in disciplinary action and liability issues.

The Three Prime Questions Framework

The Indian Health Service (IHS) developed the "Three Prime Questions" approach, which many pharmacy schools teach as a counseling framework:

  • What did your doctor tell you this medication is for? — Assesses the patient's understanding of the indication.

  • How did your doctor tell you to take this medication? — Verifies understanding of dosing and administration.

  • What did your doctor tell you to expect? — Addresses expected benefits and potential side effects.
  • Structuring Effective Counseling Sessions

    For New Medications

    When a patient receives a new medication, the counseling should be comprehensive. Cover the drug name (brand and generic), indication, dose and frequency, route of administration, common side effects, serious warning signs, interactions with other medications or foods, and proper storage.

    For Refill Medications

    Refill counseling focuses on adherence, any changes in the patient's condition, new medications that might interact, and any side effects the patient has experienced.
